will see a link to the one-page Health Questionnaire that new clients fill out
when they come in. If you are able to print this and fill it out before you
come in for your first appointment, please do. That will save you some time in
the office.

the form and then fill it out.
Remember, you will need to bring this completed form with you to my
office.
If you are unable to print it and fill it out ahead of time, I will have a copy available in my office for you to fill out when you arrive.
